Sorry. A crash in
\pdfximage height 4cm {img.pdf}\pdfrefximage\pdflastximage
\pdfximage height 6cm {img.pdf}\pdfrefximage\pdflastximage

should be avoided by
\immediate\pdfximage height 4cm {img.pdf}\pdfrefximage\pdflastximage
\pdfximage height 6cm {img.pdf}\pdfrefximage\pdflastximage
