[tex-live] tex4ht broken in TeXLice CD

if you installed MikTeX from the DVD, this is a MikTeX issue,
not a TeX Live issue.

by the way, you won't get MathML from a vanilla run of TeX4ht,
it defaults to making a bitmap image.
