Indeed, the bugginess is worse than all of this.  Experimentation reveals that even after manually moving 'mdugm.sty' into place, I STILL can't use [mdugm]{mathdesign} in a document.  I get a log sequence of things like this:

LaTeX Font Warning: Font shape `OML/mdugm/m/it' undefined

This is really all one big argument that one ought to make sure that tlmgr's backup facility is turned on.

The following used to work, up until the recent mangling of mathdesign:

1. Put the files ugmr8a.pfb, ugmri8a.pfb, ugmb8a.pfb, ugmbi8a.pfb into texmf-local/fonts/type1/urw.
2. Have mathdesign installed.
3. Maybe do updmap-sys --enable Map=mdugm.map and/or mktexlsr.
4. \usepackage[mdugm]{mathdesign} and have fun.

Now one apparently must manually move the .map file and mdugm.sty into place, but it still doesn't work.
