[texhax] left bracket not working in final

Hi All,

I've got what I'm sure will be a quick one for someone, but it's got me stumped.

I'm using the IEEEtran.cls class with amsmath and amstext packages.

In equation mode, my left brackets show up as a small box above where the left bracket should be.  Right brackets are fine.

Some clues (I hope):  
-The problem goes away when I switch to draft mode.
-The problem does not go away when I exclude the amstext and/or amsmath packages
-The text brackets work fine.
-The problem goes away when I switch class files (to a PhD class I also use)
-The problem does not occur with \big[, \Big[, \bigg[ and \Bigg[

I'm trying to use \begin{bmatrix}, or put brackets around \begin{array} -- \end{array}, but keep getting the little box.

Help!
