When I have needed a footnote in a part of a page where it would not
normally go, I have built the special footnote manually:  save the
value of the footnote counter (+1 perhaps), typeset the footnote reference
by superscripting the saved number, typeset the footnote itself with that
saved number at the beginning, and then manually advanced the footnote counter
past the special note number so it was at the right place for the next
footnote.  I'm not sure this is the best way to handle this, but
it worked.  If I had to do this very often, I would probably define
a couple of macros to typeset the special reference number and the
special footnote.
