The definitive  guide to all primitive TeX commands is _The TeXbook_ 
by Knuth.  The index indicates the primitives with an asterisk (*).  
the same book has all the plain TeX commands, some of which may have 
been migrated to LaTeX. 

For the LaTeX format I am less certain. You might have to look at the 
sources.  The Goosens book is probably the most definitive. 

I use the Kropka and Daly book.
