I've been testing the upcoming release of the Comprehensive LaTeX Symbol
List against the latest TeX Live.  Along the way I've been fixing package
incompatibilities such as the ones you reported.

The source of the problem is generally that a newer version of some package
introduces a symbol that (a) didn't exist in the previous version and (b)
conflicts with a symbol from a different package.
