[texhax] toc and pdfTeX

The short answer is that I find LaTeX far too constricting for most kinds
of work.

I do occasionally use LaTeX, but I always find that I end up: 1) having to
look up loads of things (probably mostly because I use LaTeX so rarely); 2)
getting frustrated because I keep wanting to change defaults. I generally
use LaTeX when I have to do deal with lots of floats (mostly tables and
figures); otherwise I find plain TeX far easier to deal with.

This is the first time I've had to put a TOC in a book for which I've used
TeX, though.
