[texhax] Help with Miktex

It is certainly where? In the settings window, click on Roots. Click
on "Show MiKTeX-maintained root directories". Is it below one of
those texmf roots?

If it is below one of those texmf roots, it may not be in a suitable
sub-directory.  I don't know whether MiKTeX's search is as fussy as
TeXLive's search but this might be the issue.  What kind of file
is it?
