Dear Dick,
Ali created the file under Windows/MiKTeX and said that everything
works perfectly there but he can't compile it on OS X.  Hence no
editor on OS X involved.

Unless someone proves me wrong, there is no need to blame MacTeX.

Ali said that the source file is huge and that he will send it
on request to people who are willing to help.

I asked him to send the sources as a .zip file to the list though.
