[texhax] Page number in table of contents page not where I told fancyhdr to put it

The start pages of chapters use the plain style. To change this
style see the documentation of fancyhdr "7. Redefining plain style".

You get the empty page between contents and the first chapter as the
page of your chapter is overfull and so move one page on. Reduce the
\vskip.

You only have start pages of chapters here. Add more pages and they
will have your pagestyle.
