Thanks Alan.  The problem is not with what I'm using (I don't use 
Windoze/MiKTeX at all).  And my author/friend is a private consultant 
and so is unconstrained by university IT bureaucracy.  That being said 
he may have installed a minimal version of MiKTeX and as Ulrike Fischer 
suggested may not have enabled on-the-fly installation.
