These all look fine. And now that I look back at your original message
again, I realize that the problem isn't with the .map files, it's with
locating or activating the .pfb fonts.

You could, but I don't think that will fix things.
Do you have a folder
/Library/Caches/Type1-sfnt-fonts
on your system? (This should be created by XeTeX the first time it
needs to activate a .pfb font. I'm wondering if it got created.)
What do you see as output from
ls -l /Library/Caches
and
ls -l /Library/Caches/Type1-sfnt-fonts
I'm guessing that the conversion of .pfb files for use by xdv2pdf is
failing in some way.
