I'm using two distributions on Windows: MikTeX and the stand-alone
ConTeXt distribution.
I was pretty astonished that all I had to do in order to use XeTeX
with the standalone ConTeXt was to copy the binaries from W32TeX to
the ConTeXt tree and everything worked out-of-the box. I tried the
same with MikTeX, but with no luck. The main reason is that MikTeX has
plenty of its own standards, while most other distributions are
web2c-based and mostly compatible with each other.
I don't claim that it's impossible to use XeTeX with MikTeX, just that
it might be much easier to use it on just any other TeX distribution
(XEmTeX, TeX Live, ...) but MikTeX.
