[XeTeX] No hyphenation patterns were loaded

This shouldn't be necessary. Simply check czech in the language tab of
miktex settings.

Yes. The "update formats" button updates only the formats that are not
excluded (you wouldn't want miktex to build a lot of formats that you
never use anyway...). Either build the xelatex format by selecting it
and then click "build", or "include" it.
