Can you send me (personal email) the list of files in the gentoo
texlive-2023 package (or point me where I can find that)?

THose are perl scripts, but I am also quite sure that tlmgr should NOT
be used.

That should be the shell script, as are some others.

No, it is a mixture, as Karl said.

Well, good to hear, but (1) the perl script of mktexlsr is slow and (2)
it is not well tested.
