just adding a

\AtBeginDocument{\typeout{test}}

at the very end of memhfixc shows that this AtBeginDocument gets 
executed before nameref is even loaded

This also means that the

\AtBeginDocument{%
   \@ifpackageloaded{nameref}{\let\nameref\titleref}{}%
}

is never executed
