[XeTeX] Type 1 fonts in W32 (and Linux) XeTeX

I have checked-in a set of changes to xdvipdfmx that I believe will
correct this problem; at least, it's working so far in my tests.
Those who have been seeing off-by-one glyph errors with some Type 1
fonts, please feel free to build the latest code from xdvipdfmx/
BRANCHES/macosx and let me know if this solves the issue for you.
(Yes, you read that right: get the code from the macosx branch in
Subversion, even though your target system is Linux or W32. The glyph
index correction has not been merged into TRUNK yet, though it will
be eventually if it works as expected.)
